2025 Services
Individual, Couple & Family Therapy
Psychotherapy involves individuals, families or groups talking with a professional specifically trained to guide
events toward constructive change. Therapy can be beneficial for a variety of problems and concerns which,
without treatment, may result in further impairment of functioning and decreased family cohesion and
enjoyment. The benefits of treatment can include an improved sense of well being, clearer view of choices and
goals, improved relationship satisfaction and increased ability to be productive at home, in school and at work.
At times during treatment, it may seem that the problem's impact has worsened, but this is usually temporary and
often necessary to the change process. The risks of treatment are minimal under the guidance of a properly
qualified, ethical practitioner. You are encouraged to discuss any risks in your treatment with me. You may also
want to explore the alternatives to treatment available to you, such as pastoral care, self-help groups and/or
changes in your environment which may prove beneficial.
I am a clinical social worker with over 35 years of experience treating a variety of individuals and
families. My credentials include being a Licensed Social Worker and Licensed Marriage and Family Therapist
in the State of New York, a past Clinical Member and Approved Supervisor of the American Association for
Marriage and Family Therapy and a member of the Academy of Certified Social Workers of the National
Association of Social Workers. I am a Clinical Member of the Association for the Treatment of Sexual Abusers
(ATSA). I received my Masters Degree in Social Work from Portland State University, Portland, Oregon.
My colleague Tayah Jackson, has a bachelor's degree in Forensic Psychology and a Masters in Mental Health Counseling.
She practices with my supervision. Her self pay fee is $25 per session.
Therapy deals with important personal matters and the sharing of information, with some exceptions, is
controlled by the client, NY law and professional ethics regarding confidentiality. Exceptions to confidentiality
include:
- Physical and sexual abuse of others, which must be reported to the authorities under NYS law.
- Threats to harm others, which the courts have determined should be reported both to the authorities
and the intended victim.
- Serious indications you may harm yourself, which should be evaluated by medical professionals.
Other limits to confidentiality may exist in particular cases and should be discussed. Information may be
released upon signing a Release of Information request.
Therapy is provided based on units of time, usually 60’ or 30’ sessions. Mental health treatment covered by medical insurance
is billed at $165 for the 1st appointment and $130 for ongoing sessions per hour. Missed appointments are without 24 hour
notice are billed at $50.
For treatment not covered as mental health care (couples therapy, some evaluations, family consultation,
etc.) my initial fee is $120 for one hour, payable at the time of the appointment. Ongoing session fees can be
discussed at the time of the initial session. A sliding fee schedule based on income is available. Providing
testimony for Court is billed at $1000.00 per day.

Group Therapy
Group Treatment for Troubling Sexual Behaviors
North Coast Counseling is offering therapy groups for men who have engaged in troubling sexual
behaviors such as exhibitionism, sexual aggression to adults or children and/or pornography use and
would like to end their involvement with these behaviors. The group will use cognitive-behavioral
techniques, exercises and homework to address motivations, improved behavioral control and
achieving a healthy sexual adjustment. Group members will be expected to have had a professional
sexual behavior risk assessment and be in psychotherapy with a primary therapist.
Each group runs in 10 week segments for 60 minutes. Participants are expected to develop goals for
their work in the group. The fee is $50 per session, $450 for the series if paid at the first session. A
sliding fee is available for those with limited incomes. Excellus Blue Choice and MVP provides
coverage for part of the fee.
Group for Significant Others and Loved Ones of Those Engaging in Troubling Sexual Behavior
Where do you go and who do you turn to when someone you love has engaged in troubling
sexual behavior? North Coast Counseling is offering a group for people who are grappling
with how to cope with the reality of such an occurrence in their lives. The group will focus on:
• Dealing with betrayal, anger, broken trust and grief
• Regaining trust and healing intimacy
• Reintegrating healthy sexuality and relationships
• Collaborative treatment with people who have similar experiences
We will meet in bi-weekly, 75-minute sessions
Dates: To be determined by interested parties’ schedules but intended to start in January 2025.
This is an open group and new members can join at any time. The Group will meet at
North Coast Counseling office, 277 Alexander Street (entrance on Gardner park; free
parking in lot behind building off Gardner Park), Suite 306.

SEXUAL BEHAVIOR RISK EVALUATIONS
Sexual behavior, pornography use and abuse concerns present a variety of issues that must be examined:
aggressiveness, sexual attractions, use of secrecy and denial, level of self-control, inclination to control others,
use of chemicals, mental status, and willingness to help self and others. With over40 years experience
evaluating a wide range of sexual behaviors and directing a specialized, comprehensive outpatient treatment

program, North Coast Counseling provides evaluations and treatment of offenders and their families in an
outpatient setting.
The evaluation process involves 3 clinical interviews with the individual and significant others and
administration of standardized tests. The testing may include the LOOK Assessment for Sexual Interest, the
Sexual Adjustment Inventory, the Clinical Measurement Package, and family assessment instruments. Results
of the evaluation will be summarized and provided to concerned parties and the client. The summary will
highlight relevant findings, summarize the overall functioning of the client, assess the risk of sexual offending
using accepted risk assessment instruments, and make specific recommendations as to the therapeutic changes
needed and what types of treatment could be most productive. We emphasize having goals for treatment to
maintain a productive focus for the work. We can also assess treatment progress with repeated administration
of these measures.
The evaluation requires the client to discuss the offense, his/her life circumstances and explore what changes
need to be made. We work closely with probation and parole officers, as well as child welfare agencies
regarding behavior in the community. Written consent is required to authorize release of a summary of our
evaluation to appropriate parties and permission to review police reports and victim statements. The total
evaluation process requires 6 weeks.
The fee is $1000 which covers 3 interviews, record review, testing and report summary.
A deposit of $300 is required at the first visit. The remaining fee is due: $400 at visit 2; $300 at visit 3.
The report will be released once all fees have been paid. Additional time related to the client (such as court
time) is billed at $1000.00 per day.
In the event that you wish to continue in individual or group therapy through my office, insurance may be billed
for these services depending on deductibles being met and copays are due at the time of the service.
Referral of clients can be made by calling Carl W. Christensen at North Coast Counseling. Referrals by agencies,
attorneys and courts should be accompanied by police reports and other materials relating to the sexual
behavior.
NYS SORA Re-evaluations & Adam Walsh Act Evaluations.
Individuals listed by the NYS Sex Offender Registry Act (SORA), after a period in the
community without incident, have been seeking to be re-evaluated for the SORA Board to
consider lowering their level or other administrative agencies. Also the United States
Immigration Service prohibits individuals convicted of a sex crime to sponsor an immigrant
to enter the US unless the Director determines the sponsor does not pose a risk. To
support this, we can provide a specialized sex offender evaluation using the most current
evidence-based risk assessment tools to address their risk at the time of the offending or
sentencing; and their risk after a number of years in the community.
The evaluation follows the following format:

• Has his risk changed from when the client was evaluated at
sentencing? and if it has reduced,
• Would lowering his risk level be appropriate for community safety?
This evaluation involves reviewing existing offense records, including original SORA rating
and other risk assessments. The strategy is to use our evidence-based risk assessment
protocol and rate the client twice:
• To determine how we would have rated them originally when offense occurred,
• and then re-evaluated in the present.
A brief summary is presented of the client's background, offending history and
circumstances. The risk level determined at the time with items scored that raised the rating
above Level 1.
The re-evaluation involves 3 clinical interviews, review of background material available,
administration of the testing package and production of a written report. We request 6
weeks to complete this report and the client will receive a copy. The services of an
attorney will be required for the legal filings.
The cost for this re-evaluation is $1200; half payable and the first session and the remainder at the third session. This service is not covered by medical insurance.
